What is MyChart?
MyChart offers patients personalized and secure online access to the medical record. It enables patients to use the Internet to help manage and receive information about their health. With MyChart, patients can:
- View your health summary from the MyChart electronic health record.
- Request prescription renewals.
- Access trusted health information resources.
- Share medical information with another person or entity.

MyChart is a free service offered to our patients.

This will enable you to log in and create your own username and password. If you were not issued an activation code or sent the activation request, you may call your provider's office to get one or ask to sign up during your next office visit.

How do I sign up?

For outpatient visits, results are released immediately upon completion
For inpatient visits, results are released at the day of discharge.
NOTE* A provider may not have had time to review a result before it appears in your MyChart account. We encourage you to wait to view your result until your provider has had a chance to review the same and contact you. If you have urgent concerns, please contact the office.

If some of my health information on MyChart is not correct, what should I do?
You have the right to request an amendment to your record as explained in the Notice of Privacy Practices. Fill out the Medical Record Amendment Request Form found on our website by following the Patients & Visitors tab and clicking on the Obtain Medical Records link.
You will generally receive an answer within 1-3 business days. Please note that MyChart should not be used for urgent situations. Contact your medical center if the situation requires immediate attention or dial 911 if it is an emergency.

Yes, you can. This is called proxy access. They will have to give you access to their account by selecting "Friends and family access" and modify or add new access to your health record.

Return to TopCan I ask questions regarding a family member from my MyChart account?
To ask a question about a family member, you would need to have a proxy relationship established and be able to send the message from their area. You should never ask a question about another patient from within your own MyChart account.
Return to TopNo, due to the sensitive nature of medical information, each adult must sign and submit a Release of Information request and establish their own MyChart account.

Click the "Forgot password" link on the sign-in page and follow the steps. If you still cannot log in, contact one of our outpatient clinics to get a new password.

You can call one of our outpatient clinics to have a new email or text message generated or click here to submit an online activation form.

For email changes log into MyChart and from the Your Menu selection, go to the Account Setting menu and select the Security Settings option to open the change password form.

How is MyChart secure?
We take great care to ensure your health information is kept private and secure. Access to information is controlled through secure activation codes, personal usernames, and passwords. Each person controls their password, and the account cannot be accessed without that password. Unlike conventional e-mail, all MyChart messaging is done while you are securely logged on to our website.

MyChart is owned and operated by the University of Michigan Health-West and is fully compliant with federal and state laws pertaining to your privacy. Your name and e-mail address will be treated with the same care and privacy given your health records and will never be sold or leased by the University of Michigan Health-West.

We aim to protect your privacy and security of your information. While logged into MyChart, if your keyboard remains idle for 15 minutes or more, you will be automatically logged out of MyChart. We recommend that you log out of your MyChart session if you need to leave your computer for even a short period of time.

What do I need to use MyChart?
You need access to a computer connected to the Internet and an up-to-date browser (such as Chrome, Firefox or Microsoft Edge). You may also access MyChart from an Apple or Android device using the App.
Return to TopIs my activation code my user ID?
No, your activation code is not your MyChart ID or password. You will use this code only once to log into MyChart for the first time. When you log into MyChart the first time, you will then be asked to create your own unique MyChart ID and password.
For your security, your activation code expires after 30 days and is no longer valid after the first time you use it. If you still have problems, You may contact us here to submit a MyChart support request or you can call one of our outpatient clinics for assistance.
